My belief is that women involved in this fetish are generally going to be too "little" for sex to be a factor. In the case of role-playing with my wife for example, she is a very precocious four year old. She's basically a big girl and doesn't need diapers but loves bed time stories from daddy and sleeping with her stuffed animals. My own preference in this area is much more sexual and so she will occasionally indulge me but I refuse to force anything on her in this area. She's a good enough lover in her own right despite again having a somewhat suppressed sex drive.
