First, Vickie is right about holding your urine. You can cause kidney damage and in an extreme case you could rupture your bladder.

To pee in your diaper, you have years of conditioning to unlearn and you can only overcome it by practice. Using the toilet only reinforces the conditioning, so any time you spend out of diapers slows your progress. You will have to do it intentionally at first and over time it becomes easier.

I always had problems wetting my diaper in bed because I was always concerned about getting the bedding wet. I sleep on my stomach and most disposables don't have much padding up front. I solved the problem initially by switching to cloth at night and making sure there was enough padding to absorb everything. Then, I would always drink enough water before bed to ensure I would need to pee in a couple of hours (about a quart). I would wake up around 3:00 am and wet the diaper. At first, I would have to stand up next to the bed. You can also practice during the day by laying down in the bed to wet your diaper. After 5 or 6 weeks of doing that every night I needed less and less effort to empty my bladder until I was barely waking up at all. After a couple of months, I would wake up wet in the morning and not remember doing it. I'm fairly sure I was still waking up to pee because later when I didn't sleep with a diaper, I didn't wet the bed. I didn't remember waking up because there was so little effort involved. The bonus is that I can switch back and forth without worry of wetting the bed. This is something you have to keep practicing, because if I go very long without sleeping with a diaper, I have to start over again. Being able to wet while in a car also required effort for some reason. The bottom line is practice, practice, practice.
