Yep. I guess "24/7" is not the precision definition I could have hoped for. This is definitely a "nights only" thing.
As for your scenario, I was watching some trashy documentary and the answer to at least ONE of your questions is that they cut out a wall or door to get you out (by that stage of obesity, it's usually a paramedic or an undertaker doing it).
You probably didn't need to know that.
