I am a straight male in my 20's and I stay in a lot of hotels through work, sometimes it's just for one or two nights, but it can be up to a month.
If it's a short stay, I normally don't bother saying anything, but check the mattress on arrival, if its protected or stained already, I'll wet it in the morning (I don't like sleeping in a wet bed).
If it's a longer stay, I will email the hotel before I arrive (I always book online), and explain that I have "occasional" accidents, and would prefer the bed to have a waterproof cover. I have never had any problems with this request and it means I wet as many times as I want, which is more then occasionally ;-)
I have never been charged extra and the maids have always replaced any wet sheets.
