For what it's worth - I'd try to steer clear of fantasising about somebody with a disability having accidents - but for me it's the same with anybody who is compelled by circumstances rather than volition to hold on to an overloaded bladder, or finds it emptying despite their effort. The fact that the individual is 'wedded' to the church would be a different issue. For example, Nuns are out of the frame as far as I'm concerned (would-be Nuns are a different matter) and I'd just try to switch off my imagination if it strayed that way. Perhaps I wouldn't always succeed - that's nothing to get upset about, just a matter of not deliberately thinking about that person in that way. Maybe finding an alternative 'target'. As for "Is it a sin?" Would you expect to be told in confession that it wasn't? If it's too embarrassing to confess to, that's probably an answer in itself.
