My wife, twice in about 15 years.
She gets easily constipated and once had be hospitalized because of severe constipation.
The first time we had moved abroad as expats and like often in a new environment had difficulties defecating. After a couple of days she bought a laxative which she took. Nothing happened the day after she took it. In the late afternoon she took some more, we went shopping and on the way back it started working. By the time we got back to our apartment she was desperate, she got out of the car walked with careful little steps to the entrance of the building, was squirming while waiting for the elevator and had pooped quite a lot in her panties by the time we were in our apartment.
A second time we were traveling and she could not defecate. One evening she told me she was concerned about getting constipated and added to it that once our twice a day she ended up in a situation of being able to go right away. This urge however would disappear very quickly and she could poop in her panties, isn't it. I told her to, should that happen again, just go in her panties. We could look for an opportunity to clean up as soon as possible after and nobody would have to know anyway. The next day in a public park, after having taken some distance from me, she came back and said that she had pooped, in her panties. Later she said it was difficult to walk normally with poop in her panties.
