I have a shy bladder and often take a long time to start, or I can't do it at all, whether I'm full or not very full. It happens not only in public, non-toilet situations, but also could happen to me in any restroom (work, a friend's, even my own) if people are waiting for me (even if not within earshot); if someone else needs to go in after me; if I can hear people, like at a party or restaurant, even if chances are they can't hear me; and of course, if people can hear me, it's very difficult. What helps in the restroom at work is if somebody else starts peeing, I can usually start. But if I just come in and two women are at the mirror talking or something, I have a difficult time peeing until they leave, or until someone else takes the stall next to me and starts peeing. I also have a hard time talking while I pee, unless I'm very distracted and not concentrating on the process at all. And of course, being in wide-open public situations just makes it that much worse.
